pcPix.com - Model Releases

Introduction When a pcPix.com photographer undertakes an assignment, all images produced as a part of that work are the property of the photographer and/or pcPix.com. Still, the images are of private citizens, and pcPix.com cannot by law and will not by policy use such images for any commercial purpose without the express permission of the subject or, in the case of a minor subject, the subject's parent and/or guardian. The document we use to establish this permission is a Model Release

It is not normally necessary to sign a model release if you're photographed by pcPix.com, but at times we may ask you to do so. For example, sometimes we'll want to use images in our website portfolio to show others our work; that use requires a Model Release. Certain promotions and discounts may also require execution of a Model Release.

The following are examples of our Model Releases. One form is designed for adult models, and the other for children. Should you be asked to sign a Model Release, two paper copies (one for you, one for pcPix.com) for each model to be covered by the release will be provided for your signature.

In exchange for fair and valuable consideration received, I hereby grant John Deurbrouck, his heirs and assigns the following irrevocable rights and permissions with regard to the photographs that he took of me, in which I may be included with others, on the date(s) listed below:

To copyright the images in John Deurbrouck's name, or any other name that he may select.
To use, re-use, publish and republish the images in whole or in part, separately or in conjunction with other images, in any medium now or hereafter known, in original form or modified in appearance, for advertising, trade, and any other lawful purpose.
To use fictitious names in association with such images.
My real name may be used in association with such images only as follows:
My first name may / may not be used (please select and initial)
My last name may / may not be used (please select and initial)

I hereby release and discharge John Deurbrouck, his heirs and assigns from any and all claims and demands ensuing from or in connection with the use of the photographs, including any and all claims for libel and invasion of privacy (except as provided in paragraph 4 above). This authorization and release shall inure to the benefit of the legal representatives, licensees and assigns of photographer John Deurbrouck.

I hereby affirm that I am over the age of majority and have the right to contract in my own name. I have read the above authorization, release and agreement prior to its execution and fully understand the contents thereof. This agreement shall be binding on me, my heirs and assigns.
